SEYMOUR, Ind. -Schneck Medical Center recently announced the launch of its online Community Health Needs Assessment (CHNA) and community members are invited to participate by completing a survey.

As we strive to continually improve our services and address the evolving needs of our community, your input is invaluable. By participating in this CHNA, you will share your perspectives, concerns, and priorities regarding health issues.”
A CHNA is a comprehensive process that helps identify the community’s health issues. It involves gathering data, assessing health indicators, and soliciting input from Jackson County residents. The insights gained from this assessment will inform Schneck’s strategic planning and resource allocation to better serve the community’s needs.
Participating in Schneck’s online CHNA is easy and convenient, and you can complete the survey in several ways. The survey link is available on Schneck’s website and its social media channels including Facebook and Instagram. Printed copies can be picked up at the greeter’s desk on the medical center’s main campus, Mondays through Fridays, from 7 a.m. to 4 p.m.
The survey covers access to healthcare, chronic disease management, mental health, and community resources. Your responses will remain confidential and will be aggregated with others to identify common themes and trends.
By participating in this CHNA, you are helping shape the future of healthcare in the community. Your input will directly influence Schneck’s priorities and initiatives in promoting health equity and improving the overall well-being of Seymour and Jackson County.
